SBHCs Beyond the Clinic

This module will clarify delivery of primary care in safety net settings from primary prevention to achieve population health and equity. It will also present examples of school-wide strategies to improve population health and education outcomes. The module will then discuss pathways to sustainability through documenting social determinants of health, including a discussion of coding and other strategies for increasing SBHC value. The group discussion provides an opportunity for analysis and synthesis of module content.

Learning Objectives:

  • Discuss SBHC intervention and prevention strategies "beyond the clinic."
  • Examine how SBHCs can increase accountability by documenting and addressing social determinants of health that pose risks to students
  • Summarize and integrate concepts of module segments for applicability in SBHCs

Instructions — For this module:

  • Review the module's pre-recorded presentations, which total 60 minutes.
  • Please note that review of all self-paced module content needs to be completed prior to your discussion session.
  • Participate in your interactive group discussion session by video conference for 90 minutes (attendance will be recorded).
  • Following your discussion session, complete an online proficiency assessment and session evaluation.
